PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed unused five door cooler in the rear kitchen prep area.instructed to repair or remove from the premises and maintain. 4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Instructed to detail clean the kitchen exhaust hood/filters to remove excessive accumulation of grease build-up and maintain. 5.
