Result: Fail Type: Restaurant Risk: High Violations (12): 1. FOOD-CONTACT SURFACES: CLEANED & SANITIZED: Observed residue on interior surfaces of ice machine. instructed manager to clean and maintain ice machine. 2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed several dead roaches inside drip tray of pop machine, inside cabinets under beverage area, inside bottom compartments of prep coolers, inside gaskets of reach-in coolers, on floor along walls 3. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed over 20 live roaches crawling on door gaskets of prep cooler, on slicer, on prep tables, on shelving units, on floor behind reach-in coolers in prep area, on wall and on floor along walls and 4. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed bulk containers not covered. instructed manager to cover all bulk containers to protect from contamination. 5. WIPING CLOTHS: PROPERLY USED & STORED: Observed soiled wiping cloths stored on prep tables. instructed manager to store wiping cloths in sanitizing solution between uses.
