INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed 30 or more small live flies on walls in rear prep area near in use prep tables, surfaces of dried kitchenware stored on shelving racks, in bar flying near floor drains, and on exterior surfac 3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed walls in rear prep area with chipped/peeling paint, instructed to provide smooth and easily cleanale surfaces 4.
