ADEQUATE HANDWASHING SINKS PROPERLY SUPPLIED AND ACCESSIBLE: Observed no handwashing signs or posters near hand washing sinks in front bar area and inside washrooms. m ust provide hand washing signs or posters and maintain at all times. 2. PERSONAL CLEANLINESS: Observed a few employees preparing and serving food without wearing hair restraints. must wear hair restraints at all times food is being prepared and served. 3. WIPING CLOTHS: PROPERLY USED & STORED: Observed wiping cloths stored on top of prep tables in kitchen food preparation area. must properly store wiping cloths in a sanitizing solution when not in use. 4. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Observed grease on sides of cooking equipment on main kitchen food preparation area. must clean and maintain. 5. GARBAGE & REFUSE PROPERLY DISPOSED; FACILITIES MAINTAINED: Observed outside grease container not maintained. found to have grease encrusted on top lip, lid and sides around container. also observed grease on ground sorrounding grease container. this is a food (+2 more) Address: 3555 W ADDISON ST
