ADEQUATE HANDWASHING SINKS PROPERLY SUPPLIED AND ACCESSIBLE: Found the right washbowl in both of the girl's 1st fl toilet room with no running water. instructed to provide water and maintain. 2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Found door #3 not rodent/insect proofed. 1/4" gap underneath and 1" gap in between double doors. instructed to rodent/insect proof doors and maintain. 3. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Found rodent activity on the premises. 40 mice droppings under the ramp at door #3 and 100 mice droppings in the janitorial closet. also, found potential nestings under the ramp. instructed to remove 4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Found the floors by door #3 and in the janitor's storage area with leaves, dirt and debris. instructed to clean and maintain floors. -found tissue on the walls/ceiling in the girls 2nd fl by classroo 5.
