PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing/damaged wall tiles in prep area around/behind 2-compartment sink. must repair damaged wall tiles/seal holes. 3. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed food walk in cooler/walk in freezer light bulbs not working. must provide working light bulbs for said walk in cooler/freezer.
