WIPING CLOTHS: PROPERLY USED & STORED: Found soiled wiping cloths on prep tables. instructed manager to store wiping cloths in sanitizing solution (sanotozer must be at the requird ppm) between uses. 2. SEWAGE & WASTE WATER PROPERLY DISPOSED: Found cloudy wastewater backing up from the floor drain in front of the 3-compartment sink when the water is turned on at the 3-comp sink. instructed to find source of backup, fix and maintain good pl 3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Found the ceiling in the waiting room damaged and an opening in the ceiling in the front prep over the register. instructed to fix the ceiling and cover the opening in the ceiling. maintain same. 4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Found the light shield missing in the front prep above the hand washing sink and the light shield cracked in front of the ventilation hood. instructed to replace and provide light shields. 5.
