Result: Fail Type: Restaurant Risk: High Violations (4): 1. FOOD PROPERLY LABELED; ORIGINAL CONTAINER: Observed squirt bottles and containers for dry ingredients in basement and main prep area not labeled. instructed to label with common food name and maintain. 2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed about 20 small flies and 2-3 big flies on site. observed small fly activity in basement dishwashing/ food storage area, in rear food prep area and at the bar. found an opening under the handw 3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed a black wallbase not smooth creating openings on the lower wall in the storage area near the rear exit door in the prep area. also, observed an opening in the lower wall under the handwashing 4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed the filters under the ventilation hood with build up and in need of cleaning. instructed to clean and maintain.
