INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed two live roaches on site at the time of the inspection. one roach on the lower wall under the drainboard of the dishwashing machine and the second on the floor near the small food prep cooler 2. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed food items in containers stored on the floor under and surrounding the cooking equipment under the ventilation hood. instructed to always store food items on prep table and maintain. 3. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Observed food debris on under the microwave above the food prep cooler in the rear and encrusted grease on the outerior sides of the cooking equipment in the rear food prep area. instructed to remove 4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN 5. ALLERGEN TRAINING AS REQUIRED: Observed no allergen training certificate from new certified food manager. instructed all certified food managers must take course and maintain record on site and maintain.
