INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed a 1/4" gap at the bottom of the main floor rear exit door and basement exit. must provide a tight-fitting seal at all outer openings to prevent the entry of pests. 2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: 6-501.112: **c** removing dead or trapped birds, insects, rodents and other pests observed over 50 trapped roaches on monitor traps and roach carcasses in kitchen under cold and hot holding units, und 3. IN-USE UTENSILS: PROPERLY STORED: 3-304.12: **c** in-use utensils, between-use storage observed in-use knives being stored between prep tables; instructed to properly store knives. 4. SEWAGE & WASTE WATER PROPERLY DISPOSED: Observed condensate drainage water line from walk-in cooler/ walkin freezer in beasment storage/ staff locker room discharging into plastic bucket. instructed to drain condensate drainage water from p 5. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing floor grout and damaged floor tiles in the dish/ prep areas . instructed to repair to create a smooth and easily cleanable surface. must maintain at all times.
