PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Instructed to remove from the premises a broken unused stand-up 1 door freezer unit stored in the kitchen prep area. 4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed dim lighting inside the walk-in-cooler.instructed to provide brighter light bulb and maintain adequate lighting at all times. 5.
