4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed excessive grease and food debris on floors under fryers, equipment in bar, and beverage station connected to prep area. instructed to clean and maintain.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed ventilation hood with grease, dust and dirt buildup. instructed to clean and maintain as required.
