FOOD PROPERLY LABELED; ORIGINAL CONTAINER: Observed food stored in containers in all the coolers, walk in cooler and dry storage without proper labeling of name. instructed to person in charge to provide a label reflecting the food item's name 3. INSECTS, RODENTS, & ANIMALS NOT PRESENT: 6-202.15 observed a 1/4" gap at the bottom of the rear exit door. must provide a tight-fitting seal at all outer openings to prevent the entry of pests. 4. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed evidence of live roach activity on premises. observed approximately 5 roaches on premises on wall above handwashng sink, on floor behind reach in cold hold unit, under oven in main prep area 5. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed roaches on floor in rear near freezers, under dry storage rack with aprons, on floor in corners near hot cooking equipment. instructed to remove detail clean, sanitize, and maintain.
