PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-201.11: observed a hole in the lower wall behind the hot water tank in the hot water tank storage closet in the rear. instructed to seal mentioned hole to eliminate potential harborage points for pe 3. ALLERGEN TRAINING AS REQUIRED: 2-102.13: observed no allergen from the food manager with the servsafe certificate on stie at this time. instructed all food managers must take course and maintain record on site.
