3. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: Observed cardboard on bottom shelves of display coolers. instructed manager to remove all cardboard from coolers.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed soiled carpet and unfinished floors in sales area. instructed manager to remove soiled carpet and make all floors smooth and easily cleanable.
