1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: 6-202.15 observed both rear entrance and rear exit doors wide open during time of inspection. instructed to enclose and protect outer openings.
2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-201.11 observed floors throughout premises in disrepair. raw exposed concrete in various sections. instructed to re surface floor so as to be non-porous, and easily cleanable.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-501.13 observed floors with excessive amounts of feathers, blood and chicken waste. instructed to clean and maintain throughout operation
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-501.12 observed exterior cages space where chicken feed is located accumulated with chicken waste and blood. instructed to remove, clean and sanitize at frequency.
