2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed a missing ceiling tile above the pizza prep cooler in the rear food prep area. instructed to provide ceiling tile and maintain.
3. ALLERGEN TRAINING AS REQUIRED: Observed no allergen training certificate from food manager on site. instructed all certified food managers must take course and maintaina records on site.
