2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: 6-202.15: observed the bottom of the rear exit door furthest away from the service area with openings. instructed to rodent proof and maintain mentioned door.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-201.11: observed carpet on the floors in the rear storage area furthest away from the service area. instructed to rmeove to make floors smooth and easily cleanable.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: 6-303.11: observed toilet room and rear storage area furthest away from the service area with no light. no light due to power shortage, per person in charge. instructed to restore light and maintain.
