1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed gap opening at bottom center of rear exit doors.instructed to make door tight fitting all across the bottom to prevent pest entry and maintain.
2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Instructed to detail clean and organize the rear storage room and remove unnecessary articles due to potential pest harborage and all other need items must be elevated off the floor at least 6" inches
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing ceiling tiles inside the butterflies classroom.instructed to provide noted missing ceiling tiles and maintain.
