4. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed no covered waste receptacle provided in the unisex employee washroom. must provide covered waste receptacle and maintain at all times.
5. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed dirt and grease build up on floor around utility mop sink and throughout rear food storage area. must clean and maintain. observed a few damaged floor tiles in rear food storage area. must r
