2. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed food items not covered in reach-in cooler. instructed manager to properly cover all food items to prevent contamination.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed soiled water in mop pail. instructed manager to discard all soiled water and hang mop head to dry to prevent insect breeding.
