2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed loose handwashing sink in third unisex washroom. instructed manager to properly secure handwashing sink.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed debris inside floor drains in prep area. instructed manager to clean and maintain all floor drains.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed no mechanical ventilation in washroom. instructed manager to provide and maintain.
