3. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed no self-closing door device installed at the washroom door. management instructed to install.
4. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Must provide a covered waste receptacle in the washroom.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed no working mechanical ventilation fan in the washroom. management instructed to install.
