2. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed no covered receptacle in washroom. instructed manager to provide covered receptacle in washroom.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed damaged floor tiles in sales and prep area. instructed manager to replace damaged floor tiles in all areas.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed ventilation fan not operable in washroom. instructed manager to repair ventilation fan in washroom.
5. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Observed no food handler training for employees. all employees handling food and/or equipment must provide valid food handler training.
