Violations on last inspection

1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed the rear exit doors in the kitchen and leading to the playground with a 1/4" gap, making doors not rodent/insect proofed. instructed to rodent/insect proof doors and maintain.
2. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed a leak at the base of the right faucet on the 3-compartment sink. instructed to repair leak and maintain good plumbing.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed the floors in the laundry and storage room with dirt/debris. instructed to clean and maintain floors.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed the kitchen with inadequate lighting. instructed to replace the blown bulbs in the kitchen and maintain adequate lighting.
