1. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed exposed handwashing sink draining slow in the front prep area next to hot holding unit. must repair and maintain.
2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed peeling paint on wall behind bulk food storage containers in rear food storage area and wall inside women's washroom must repaint. walls must be smooth and easily cleanable surfaces.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed clutter and unnecessary items on the 2nd floor food storage area. must remove clutter and unnecessary items to prevent rodent harborage.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed dirt and food debris on floor underneath food storage shelving inside walk-in cooler and underneath shelving on the 2nd floor. must clean and maintain at all times.
