1. FOOD PROPERLY LABELED; ORIGINAL CONTAINER: Observed bulk food container with no label stating common name of ingredient. instructed person in charge to label all bulk ingredients not in original container.
2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed fly paper in rear prep area and rear staff restroom. instructed person in charge to remove and request insect control device from licensed pest control technician.
3. UTENSILS, EQUIPMENT & LINENS: PROPERLY STORED, DRIED, & HANDLED: Observed large pots in rear storage not inverted. instructed person in charge to invert all kitchenware (pots/pans) while not in use to prevent contamination.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed broken wall tile at base of wall under handsink nearest dish washing area. instructed person in charge to repair and maintain.
5. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Observed no food handler certificates on site at time of inspection. instructed person in charge to direct all food handler employees to obtain training and retain certificates on site.
