1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed small gap at the bottom of front entry door about 1/4 inch. instructed to repair as a means of preventing possible pest and or insect activity.
2. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: Observed all cutting boards in need of replacement. instructed to replace all.
3. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: Observed the shelving in the walk in cooling unit in need of a detail cleaning. instructed to clean and maintain.
4. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Observed the inside of the walk in cooling unit and the freezer floor in need of a detail cleaning. instructed to clean both and maintain.
5. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed the one compartment sink in the food prep area with a loose faucet and a small leak as well as observed a loose faucet and small leak at the base of the faucet. instructed to repair and main
