2. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed stained ceiling tiles in first floor prep area. instructed manager to replace ceiling tiles and maintain.
3. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Observed expired food handler training for employees. all employees handling food and equipment must provide valid food handler training.
4. ALLERGEN TRAINING AS REQUIRED: Observed expired allergen training for certified on duty. all certified managers must provide valid allergen training.
