2. PERSONAL CLEANLINESS: Observed food handlers not wearing hair restraints. all food handlers must wear effective hair restraints.
3. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed no backflow device on utility sink. instructed manager to provide backflow device and maintain.
4. GARBAGE & REFUSE PROPERLY DISPOSED; FACILITIES MAINTAINED: Observed trash and debris on ground around dumpster. instructed manager to clean and maintain outside dumpster area.
5. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing and loose baseboards in dishwashing area. instructed manager to replace/repair and maintain baseboards
