4. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed the unisex employee/customers washroom door not self closing. must provide or install a self closing door device and maintain at all times.
5. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed accumulated dust build up on ceiling ventilation vents in front dining area. must clean and maintain. observed a few water damaged ceiling tiles in front dining area and rear prep area. must
