1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed gap opening 1/4" inch at bottom of front main entrance door.instructed to make door tight fitting all across the bottom to prevent pest entry and maintain.
2. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: Observed raw wood shelves in the rear storage area.instructed to paint or seal the shelves and maintain.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed dirty floors with debris underneath the kitchen dishwashing 3-compartment sink area.instructed to clean floors in detail and maintain.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Instructed to clean the kitchen exhaust hood/filters to remove accumulation of hanging dusts and maintain.
