3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-501.11: observed cracked and damaged left light shield in walk in freezer. instructed to replace said light shield in walk in freezer and maintain.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: 6-501.12: observed ice buildup on left condensor line in walk in freezer. instructed to remove ice from said condensor line in walk in freezer and maintain.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: 6-303.11: observed non working middle light in walk in freezer. instructed to repair or replace said non working light in walk in freezer and maintain.
