1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Found a crack in the bottom of the rear exit door; instructed to seal so that door is completely rodent proofed.
2. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Instructed to clean the shelves inside of the walk-in cooler to remove rust from shelves.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Instructed to clean ceiling vents in kitchen and light shields.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Instructed to replace stained ceiling tiles in kitchen.
5. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Instructed that all food handlers must obtain the required food handler training.
