1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed openings at base of rear exit door. instructed to seal all outer openings to prevent pest entrance.
2. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed faucet attachment installed to the right of handwashing sink faucet used for obtaining filtered water attached to water line of handwashing sink. instructed person in charge to remove faucet
3. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed door of all gender employee washroom to not be self closing. instructed to install self closing device on all gender employee washroom door so that it shall be tight fitting and self closing.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing or damaged tiles on wall above handwashing sink. instructed to smoothly replace all damaged and missing wall tiles in said area for ease of cleanability.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed dust buildup on interior and filters of hood in front prep area. instructed to clean and maintain. observed dust covered ceiling and wall vents throughout facility in such areas as prep and
