3. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed no covered receptacle (broken lid) in women's restroom. instructed person in charge to provide covered receptacle and maintain.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed boxes and bags of food items in storage/hallway area. instructed person in charge to organize and elevate all items at least six inches from floor and maintain.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed filters in hood above cook line in need of cleaning. instructed person in charge to clean and maintain.
