1. PERSONAL CLEANLINESS: Observed the cook without proper hair restraint preparing food. instructed to provide and maintain at all times in food prep areas.
2. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Observed the interior of the 3-door cooler, prep coolers dirty. must clean to remove spills and debris.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Floors need cleaning along the wall base behind the cooking equipment to remove debris.
4. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Observed employee/server without his food handler training as required by idph. instructed to comply or citation will follow.
5. ALLERGEN TRAINING AS REQUIRED: Observed the certified manager allergen training certificate expired. instructed to provide and maintain at all times or citations will follow.
