2. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed receptacle in women's toilet room without covering. instructed to provide.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed hole in wall corner underneath 3 compartment sink in between wall base.instructed to seal/enclose in order to prevent pest entry
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed grease and food debris on floor under all heavy equipment in front prep along wall as well as rear storage under shelving units. instructed to clean and maintain
