1. FOOD PROPERLY LABELED; ORIGINAL CONTAINER: Observed no label on bulk container (flour) in rear dish washing room/prep. instructed to provide common name on container to properly identify.
2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed dead flies on wall by hand washing sink at main prep area and along the ledge by the windows in the dining area, and by the ledge next to garbage can in dining area. must remove.
3. NON-FOOD/FOOD CONTACT SURFACES CLEAN: Observd exterior of chest freezer located next to fryer with heavy grease spills. must clean and maintain.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed heavy grease and dust build up on filters at ventilation hood. must clean and maintain.
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed burnt out light at ventilation hood on left side. must provide adequate lighting.
