1. FOOD PROPERLY LABELED; ORIGINAL CONTAINER: Observed food storage containers in rear walk-in cooler without labels. instructed to label all food items once removed from original packaging.
2. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed missing splashguard at rear handsink next to prep table. instructed to install a washable splashguard.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed exposed foam-like caulk on the walls and around pipes in main prep area and in basement. instructed to resurface area to ensure a smooth and easily cleanable surface.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed the ventilation hood and filters above the cooking equipment in need of cleaning. instructed to detail clean and maintain at all times.
