4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Floors in rear area by dish machine in need of cleaning to remove build up instructed to correct and maintain
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Hood vent filters in need of cleaning to remove grease build up. instructed to clean and maintain
