1. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: Observed a torn door gasket inside of the two-door tall refrigerator at rear prep/dishwashing area. instructed to replace the torn door gaskets and maintain.
2. FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED: : observed rusty food storage shelving in rear prep/dishwashinga area. must repaint or replace
3. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed exposed hand washing sink draining slow in the rear prep/dishwashing area. must repair and maintain at all times.
4. TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED: Observed womens washroom door not self closing. must provide a self closing door device and maintain at all times.
5. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed missing floor tile in front kitchen food preparation area. must repair or replace and must regrout floor in rear food prepartion area and maintain at all times.
