3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed stained ceiling tiles in men's washroom and dining area. instructed to replace all stained ceiling tiles in facility and maintain.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed dimly lit prep/dishwashing area. instructed to increase light intensity in prep/dishwashing area.
5. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: At time of inspection facility provided no food handler training certificate for food handler. instructed to provide valid food handler certificates for all food handlers.
