1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed gap opening approx:1/4" inch at bottom left corner of the front entrance door.instructed to make door tight fitting all across the bottom to prevent pest entry and to maintain.
2. CONTAMINATION PREVENTED DURING FOOD PREPARATION, STORAGE & DISPLAY: Observed some food stock items stored on the floor in storage closet.instructed to provide proper, raised shelving to store stock items off the floor at least 6" inches to facilitate cleaning and main
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Instructed to remove broken/unused soda cooler stored in the customer area and maintain.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed dirty floors with litter debris underneath the grocery storage shelfs.instructed to clean floors in detail and maintain.
