2. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Observed opening in middle of double doors of door 4 in lunchroom. instructed to seal all outer openings to prevent pest entrance.
3. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Observed leaking hot water faucet at northside serving line handwashing sink. instructed to repair and maintain.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Observed stained ceiling tiles in front prep area with serving lines. instructed to replace all stained ceiling tiles in front prep area with serving lines and maintain. observed stall number #6 in g
5. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Observed one non working light in hood. instructed to replace/repair non working light in hood and maintain.
