1. INSECTS, RODENTS, & ANIMALS NOT PRESENT: Found the front door in the infant's room with 1/4" gap underneath and is not rodent/insect proofed. instructed to rodent/proof door and maintain.
2. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Found a leak at the base of the faucet on the 3-compartment sink. instructed to fix and maintain good plumbing.
3. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Found the mops stored inside the mop bucket. instructed to store hanging up for proper air drying. maintain same.
4. ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED: Found inadequate lighting (very dim) in the kitchen and kids toilet room. instructed to provide adequate lighting to both areas and maintain.
