3. PLUMBING INSTALLED; PROPER BACKFLOW DEVICES: Found no back flow prevention device on the rear mop sink. instructed to provide the device and maintain.
4. PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN: Found mops not properly stored when not in use. mop stored on the floor next to the mop bucket full of dirty water. instructed to store mop hanging up for proper air drying. maintain same.
5. ALL FOOD EMPLOYEES HAVE FOOD HANDLER TRAINING: Found food handler on site with no food allergen training certificate. instructed to have food handlers obtain and maintain a valid food handlers certificate.
