1. Failure to make a good faith effort to inform smokers or electronic cigarette users of Smoke-Free Air Act ("SFAA") prohibitions
2. Warning sign re dangers of hookah smoking not posted in each room or area of Non-Tobacco Hookah Establishment (NTHE) where smoking is allowed
3. Cold TCS food item held above 41 °F; smoked or processed fish held above 38 °F; intact raw eggs held above 45 °F; or reduced oxygen packaged (ROP) TCS foods held above required temperatures except during active necessary preparation.
4. Flavored tobacco products sold, offered for sale
5. Food Protection Certificate (FPC) not held by manager or supervisor of food operations.
